Side effects included a mean 2.7% weight gain compared with controls and leg edema, but the small sample size of included trials (n = 516 patients) prevented evaluation of more serious adverse effects. Since there is no data showing improvement in clinical outcomes such as development of ascites, encephalopathy, need for liver transplantation, or liver-related death, a related commentary (see below) suggests it is premature to recommend pioglitazone in the general management of this condition.
